The cohort’s money is parked in the health aisle this quarter. A joint replacement is one of its largest single bills.
Medicare kept a list for decades called the Inpatient-Only list. It named the surgeries the program would pay for only inside a hospital. A joint replacement sat on it for years.
Now the government is taking that list apart, one year at a time.
The timing reflects demographics. Millions of Americans are reaching Medicare age.

Where it now gets done
Medicare added 276 procedures to the surgery-center list for 2026 and began removing the hospital-only requirement. Most joint replacements are now expected outside the hospital.
So the government did the math too. Two hundred eighty-five procedures left the hospital-only list this year. Most of them are the bone-and-joint operations your generation is lining up for.
They can now be done in a surgery center. That is a small facility built for same-day operations, with no overnight stay. You are home for dinner.
For a joint replacement, the address used to be the hospital, and only the hospital. This year that changed. Someone was already standing at the new address.
The rule that opens the door also trims what anyone can charge to walk through it.
Medicare is steering these operations to the surgery center for one reason. It costs the program less there.
The agency is also equalizing what it pays across settings, a policy called site-neutral payment. A new payment model, mandatory since January, now holds hospitals responsible for the full cost of a joint-replacement episode.
So this is a volume business on thin, regulated margins. The good news sits on your side of the table. The same move that helps the operator lowers your out-of-pocket cost in the surgery center.

The list keeps shrinking on a calendar. More procedures leave the hospital-only column next year, and the year after that. By 2029 the column is meant to be empty. So watch the address this time. That is where the money in this aisle is heading. The knee is your generation’s. The building waiting for it belongs to someone who read the same calendar you can.
